While the VA Tampa Healthcare System was the only facility to reach the 90% compliance benchmark, Martinsburg VA Medical Center recorded the second-highest compliance rate at 82%.

Analysts reviewed 50 randomly selected electronic health records at each facility, including Martinsburg, using a 90 percent compliance benchmark. Nine out of the 10 inspected facilities failed to hit the 90 percent mark for documenting medication risk-benefit conversations before administering new central nervous system treatments.
